Yes, you have the option to upload your form and the Formitize team will build it for you. As part of the Free Trial, we can build your first form as a proof of concept for free. Subsequent work may incur a charge, upload a form to the team here through your account they will be in touch with you after you upload it to explain and discuss the next steps or email production@formitize.com.
The Form Bank is also available with a range of forms that may suit your needs. Web Portal > Forms > Form Bank > select the form that you would like to add to your templates > Upload Template. From the form templates you will now be able to edit the form to further suit your needs.
