Form conditions are rules set to automatically Trigger your Form Actions. It will pull against the different form field names. You are able to have multiple conditions.
For example a question in your form may be the level of severity of an injury. It may be Minor / Serious / Catastrophic. With this as a question in your form (multiple choice) you can set a form action to have a condition that if the questions is answered as 'Minor' only the health and safety team will be advised where as if is is answered as 'catastrophic' the health and safety team and management teams are advised.
