A slider field is a basic measuring/rating tool that can be added into a form. You can use it to give a numerical value to a query by sliding the measure horizontally along a line of your preferred allocated measurement that you set in the Field Settings.
The slider will display in the form like this:
In the PDF Report, it will display as the numerical value.
