Locate the Webform Security settings in the Web Portal, by going to Forms > Existing Templates > open target template > scroll to the bottom of the Details page > select the Security tab under 'Webform Options' > select SMS/Email verification in the Webform Security dropdown > enter/select Contact details > Add > Save.


Please note: In order to set up SMS verification, you will need to have the SMS feature set up on your Formitize account. (How do I set up the SMS feature?)


Or, in an individual form, when in the form, select the checkbox beside 'Convert to Webform?' > Save > select SMS/Email verification in the Webform Security dropdown > enter/select Contact details > Add > Save.




You can also set up more than one Contact to receive the verification information. Once you have selected Add, the contact information you have just added will move to a table, and you will be able to fill out the details for another. You can also delete a Contact from the list by clicking on the Delete bin beside it.




When the webform link is used, the person attempting to access it will need to enter either the phone number or email address that the webform has been assigned to in order to view and complete the webform.



Note: Anyone who attempts to access it with a different contact number or email address will not be able to receive a code., or access the webform.



Once they have successfully requested a code, they will receive it via the designated method, and will then be able to access the webform upon entering the code and pressing Submit.
